- You have a right to participate and develop your own treatment plan.
- You have a right to participate voluntary in and to consent to treatment
- You have a right to object to or terminate treatment
- You have a right to see your own records
- You have a right to receive clinically appropriate care and treatment that is suited to your needs
- You have a right that treatment will be conducted skillfully, safely, and humanely with full respect for their dignity and personal integrity.
- You have a right to be treated in a manner that is ethical and free from discrimination
- You have a right that all sessions will be confidential
- You have a right to be informed of expected results and possible adverse consequences
- You have a right to ask for a new therapist
- You have a right to rescind any releases of information that you may have signed
